• To complete an eligibility packet, and begin practicing athletics with us, please complete the following Student Eligibility Form created by the school district. Follow the instructions to process the form. Be aware that sports physicals will need to be uploaded and signed, and a copy of the form must be turned into the front office. 

                                                        

    For a "Physicians Clearance Form", please download it from the eligibility packet link provided above. Once the packet is completed, and you click "save form", it will email you a copy. Only click "submit" if the form is 100% complete, including the attached Physicians form. 

    Once the form is completed, and you have clicked "submit", see our Athletic Director, at Greenwood to ensure that you are cleared to play. If you have any questions, please contact the Athletic Director at Greenwood
